These trials tested various interventions designed to improve mental or physical health through the lens of positive psychology. One 12-week, spirituality-based digital intervention resulted in some of the most significant data points in the review: a 7.6-point reduction in systolic blood pressure measured with a standard cuff, and a 4.1-point reduction in central systolic pressure, which is measured in the aorta as it leaves the heart. These metrics are critical, as central systolic pressure is often considered a more accurate predictor of cardiovascular strain than the readings typically taken at the arm.

The most profound behavioral shifts were observed in an eight-week program delivered via WhatsApp. This program combined weekly educational sessions with daily "microtasks," which encouraged participants to make incremental changes to their daily lives. These tasks were specifically aimed at increasing physical activity, improving dietary choices, and ensuring strict adherence to prescribed medications.
Other programs focused on motivational interviewing saw similar success, with some interventions helping cardiac patients increase their physical activity by approximately 1,800 steps per day. While mindfulness programs were highly effective at improving diet and activity levels, the study noted that they did not always produce the same high level of medication adherence as the more task-oriented, structured interventions.
"The therapeutic dose that was most consistently linked with improvements in blood pressure, inflammation, and endothelial function was daily practice reinforced by weekly sessions over eight to 12-week periods," Hernandez said. "Therapeutic dosing typically involved high-frequency dosing over this time period to obtain short-term physiologic benefits, while ongoing less-intensive contact may be needed to sustain behavioral change."
